This form collects your full name, photo, phone number, and email address and is used solely for organizing the event and facilitating post-event mutual matches.Here’s how your information is used:- Before the event, your name and photo ONLY are included on digital Score Cards that allow participants to select who they’re interested in after each round.- After the event, if a mutual match is made (i.e., both parties selected each other), only then will your phone number and email be shared—privately and only with that mutual match via email.- We NEVER share your information with any third parties, affiliates, or the entire attendee list.Completion of this form is mandatory to be included in the mutual matching process.
